Medical Case Management

Medical case management services are similar to care coordination services and provide a range of services that link clients with health care, psychosocial (support groups), pharmaceutical (HIV medications) and a variety of other services.  Coordination and follow-up of medical treatment, as well as treatment adherence (sticking to your doctor’s orders), are components of medical case management.
